Micrologix 1400 with 2x 1762-0f4 only getting 3v from output of analog cards

Im very new to programing but have ran into a strange issue where i am only seeing 0-3v from my 1762-of4 cards and is the smae on both cards. The stranger part is it has been like this for a couple weeks and while connected into the plc last night with my laptop it began sending the full 10v without me knowingly changing anything and seemed to remain "fixed" after multiply power cycles and run cycles. After sitting overnight on powerup we reverted right back to only 3v.
