Had a block last week that came in that had the DB-50 pin connector on the I/O block soldered upside down. This caused the wiring to be: Pin#1 to Pin#50 on external device Pin#2 to Pin#49 on external device etc... This let the magic smoke out and damaged a high dollar servo controller. Watch these blocks if you purchase them. Phoenix of course is taking care of the issue, but double check.

Wow, too bad the pin configuration is symmetrical. If it wasn't, you would not have been able to plug it in with the shell being upside down like that.
